The Opportunity

Robinvale District Health Services' is seeking an experienced and innovative Nurse Practitioner to join our dynamic Community Health team. In this full-time role, you will be responsible for providing comprehensive primary healthcare services to the diverse population of Robinvale, Victoria.

Key Responsibilities

• Assess, diagnose, and manage a wide range of acute and chronic health conditions in the community setting
• Provide health education, promotion, and disease prevention services to individuals and groups
• Collaborate with other healthcare professionals to develop and implement holistic care plans
• Participate in the development and delivery of community-based health programs
• Contribute to the ongoing evaluation and improvement of community health services
• Maintain accurate and comprehensive patient records in accordance with professional and organisational standards

What We're Looking For

• Current registration as a Nurse Practitioner with the Nursing and Midwifery Board of Australia
• Significant experience in community or primary healthcare settings, with a strong understanding of population health principles
• Excellent clinical assessment, diagnostic, and management skills
• Proven ability to work collaboratively within a multidisciplinary team
• Strong communication and patient education skills
• Excellent communication and interpersonal skills to effectively collaborate with patients, families, and the multidisciplinary team
• Demonstrated ability to work independently and make autonomous decisions
• Commitment to evidence-based practice and continuous professional development
